  • Name of Esthetician
  • Spa Name (if applicable)
  • Spa Address & Phone #

 

Step 3 - Verification. It may take up to 24 hours to receive a response for receipt of your email. It may take several days to verify your visit. 


Step 4 - REWARD TIME! Once verified, we'll send you an email with a coupon code for $30 to spend on LeMieuxSkincare.com.**
